Seeking Help Dial Railwire through Ubuntu

I was told by a customer service person to use ethernet cable directly from the ONU and dial it through my computer. I have 18.04 on my PC. I figured out the method to start adding a PPPoE connection. But I only have my ISP ID and password. There are other 3 fields as seen below. Any idea what to enter in them ? Any help will be great.

It won't allow to continue unless all 5 boxes are filled.

They use railtel as Service name. You should confirm the rest with your LCO.

Put your user ID as username and password in the password field. service name can be anything like "railtel" did you try that?
